Extension regions may be confusing on a single page master.
So far everything is computed relative to the beginning of the page.
What about using multiple region bodies of XSL-FO 1.1?
That solution would give users more freedom but
would require them to impose alignment constraint among regions.

The description for this bug is mistaken that 'everything' is computed relative
to the page.

'change-bar-offset' (http://www.w3.org/TR/xsl11/#change-bar-offset) is computed
relative to "the edge of the column area containing the text that is marked as
changed", and the direction of the 'change-bar-offset' is such that "[a]
positive distance is directed away from the column region and into the margin".

So there is a precedent for defining a distance relative to an area rather than
relative to the page.
